The 2026 Lepton Health Equity Index ranks U.S. states and select global markets on a unified composite of healthcare access, financial protection, and outcomes equity — using the spatiotemporal methodology developed in our peer-reviewed work on “health insurance deserts.” This year’s report highlights three patterns: a widening rural-urban access gap, narrowing racial outcome gaps in select states, and growing financial protection vulnerability among informal-economy workers.
